Covered Services

• All FDA approved birth control methods, devices and supplies ( i.e. Birth control pills or patches,
  condoms, diaphragms, IUD’s )
• Male and Female sterilization procedures, e.g. tubal ligation
• Emergency contraceptive services and follow-up care
• Counseling, preventive screening and family planning options before pregnancy.

These services are also covered when in conjunction with a family planning visit

• Pregnancy testing and counseling
• Screening for sexually transmitted diseases ( STD’s ), cervical cancer, and urinary or female-related infections.
• Counseling services related to pregnancy and informed consent, and STD/HIV risk counseling.
• Comprehensive health history and physical examination, including breast exam and referrals to primary care providers. Mammograms are not included.

Note: Termination of pregnancy, treatment for STD’s, treatment for infertility, and follow-up care and not related to related to family planning are not covered under FPBP.

FHBP Eligibility Criteria

• RESIDENCE (ONLY within the five boroughs)
• IDENTITY (Non-expired Photo ID)
• CITIZENSHIP/INMIGRATION STATUS
• INCOME
• NO RESOURSE TEST

Residence:

• Applicants must reside in NYC in order to apply in NYC
• No Durational Requirements
• Common Forms of documentation include: Lease, Rent receipt, Utility bill or Postmarked mail (no more then 30 days from your appt.)

Identity:
Non-Expired Photo ID

• US Passport *
• Passport from foreign country
• Naturalization Certificate *
• Certificate of Citizenship *
• Legal Permanent Resident Card *
• Drivers/Non-Drives license
• Photo I.D. issued by an employer, school or governmental agency
